Can I (How?) put a form in a product description?

I am having problems with a product description that requires both attributes and user input.

 

The product is hand-crafted baby shoes that have personal inscriptions. I need a way for the customer to input certain data such as the baby's name, time of birth, weight, etc and have that information show up in the shopping cart at check out so that when I recieve the order, I have the customer's "personalization info" as well as the type of shoe and color they want.

 

I tried doing this using just attributes, but there is no way for the customer to input personal data.

 

Anyone have any ideas? I'm very new to php and am terrified I'm going to break osCommerce if I try anything "rash" :-)

 

Any help would be greatly appreciated!
