Credit Class and Gift Voucher/Coupon V5.21 contibution for osCommerce RC2/RC2a

Hi, I am looking for some guidance if you have the time.

 

I have installed Credit Class and Gift Voucher/Coupon V5.21 contribution for osCommerce RC2/RC2a into a site along with Open Page Checkout.

The OPC went in first and that seems to work fine.

Today I finished putting in CCGV 5.21 and that too seemed to go in ok except I have an error somewhere.

 

When I go into the Admin Area, Modules - Order Totals page, it isn't loading fully. I don't get any errors but when I look at the "view page source" from my browser, the last entry is this:

Does anyone know how/where to look to find what could be causing this? All other pages in the admin area work fine.

 

On the Modules - Order Totals page, I see the header, left column and list of modules, but no right column (to install modules) and no footer.
