Extra Images v1-61, error that destroys pages with extra images

I haven't done anything to the code or anything, I installed everything as is from the add-on, but my page now looks like this.....

The right column ends up on the left, the product and description etc end up where my logo was... and at the cottom it says "error on page"

 

Surely I haven't been the only one with this issue as I didn't change it at all.......?

You are missing the code in RED

If I suggest you edit any file(s) make a backup first - I'm not perfect and neither are you.
