country field in order_info.php

hi guys I have a small probelm which i need your help.

 

I have used Mark's "purchase without account 1.2". Since Mark is busy for sometime i am posting the problem here.

 

the orderinfo.php is no parsing correctly. i found that the page got stuck at lines

 

SelectedCountry = theForm.country.options[theForm.country.selectedIndex].value;

<?php tep_js_zone_list('SelectedCountry', 'theForm'); ?>

resetStateText(theForm);

}

 

so i removed these lines and the page is getting parsed, but the country text field is missing.

I just need to know how to get the textfield back.
