Adding Input fields to a simple shipping module

I'm sure this is an easy request, but I've looked around and I can't seem to find anything relating to it.

 

I need the ability to allow the customer to enter their shipping account info so they can be charged the shipping cost directly to their FedEx, UPS, DHL account. Basically I need a shipping module that has 2 fields (Carrier Name and Account Number).

 

I'm using the Local Delivery Option shipping module and the Institutional Purchase Order Number payment module. I tried tearing apart the payment module to see how to add input fields into the shipping module, but something wasn't right. I would have posted the code I was torturing, but I'm afraid the code is a little frankensteinish. :blush:

 

I have read many forums on here and I know someone could throw something together to get me start, that, I would be very grateful.

There is a contribution that allows you to have extra fields when the customer creates an account.

You could ask for that information when they create an account or see if you could merge that onto the shipping page, instead of the create account page.
