How to add a input field in Payment Module

I'm using the Link Point Card Services V2 payment module and was wondering how I might go about adding an input field to the module, specifically a textbox where the customer would enter the phone number of their credit card company. The number would then be stored in the database in case the transaction needs to be verified.

In case anyone finds this thread, an input for a card issuer phone number isn't really necessary. The Visa and MC merchants helplines can provide you with the phone number for any domestic card.
