Ship in Cart questions

Hello.

 

I'm having just a couple of nagging problems with the wonderful Ship in Cart contribution:

• The "Selected" icon (green checkmark) fails to appear (though it is set to "True" in the administration area)
  
• "Item Weight" is included in the estimate listing (even when disabled in the administration area)
  

I have renamed, inserted, and uploaded as instructed, but still can't get these two features to work. Any help that anyone may provide would be most appreciated!
