Can not add product attributes!!

Hi all, I found I can not add product attributes via admin section. i can greate product options and option values but when i trying to produce product attributes by pressing 'insert' a error page jump out and says:

 

1136 - Column count doesn't match value count at row 1

 

insert into products_attributes values ('', '28', '7', '16', '', '+')

Anyone know how to fix it please? Thanks a lot!
