I've found theres a bit of a bug with the e:mail system, it's been posted here a few times, but not everybody has the problem. Sometimes e:mails don't get sent if you have different e:mail addresses in the store config. Try the above, but if that doesn't work, change all the references to e:mail address to the same e:mail address. Julian oh, and enable send e:mails if you haven't already.
