Your formula looks correct.
I am assuming that your products weights are correctly done - if so,
Have you picked the correct "Table Method" (Based on weight)

Use a Table Rate shipping module. In your example above the formula would be:
4.50:1.99,9.00:3.99

Next block down, activate the "Price" radio button
