Coping with COPPA

I recently learned of the COPPA requirement when dealing with information of minors (under 13 years of age). I have done a search on this forum and there are a few brief mentions of COPPA but can anyone post a url to a kid safe site that is already COPPA compliant and/or a smaple Privacy statement covering the COPPA requirement? What I'd like to do is have a site that sells services (classes) ie: karate, music, dance, cheerleading, mommy & me , etc for all ages (from newborns to Adults) ....

Information that we currently collect (on paper) at time of enrollment :

Student Name:
Address:
Hm Phone:
Birthdate:
Age:
School:
Mother's Name:
Father's Name:
email address:
medical conditions we should be aware of:

Would it be wise to allow online enrollment for classes as a couple of our competitors have done or just offer a brochure or catalog of classes and make them come to location in person to enroll?

As far as process goes, should we require the adult to sign up as the
primary account holder, and require them to enter the child's information,
including birthdate, and their relationship to the child, along with a
checkbox that certifies that they read and understood the privacy policy? Would that cover the site for COPPA?

This would complete the requirements of COPPA if you also have the parent fill out a form and fax it to you stating they have allowed you to collect this information.
