I need to be able to have attributes selected like this:

Tshirt
Kids Tshirt
Mens Tank Top

Then after the first attribute, a size needs to be chosen

I don't want a size to be chosen without a shirt, and I DO need a size choosen once a shirt is chosen.

Did you go and select the attributes you want attached to each item. You have to do that.
Also, if you have not added any attributes handling options try this one
