It happens when an admin user opens an order in the admin tool and then gets distracted for a while (say 30 mins). When they return and (for instance) write a message to the cutomer and change the order status, when they try and save this the comment is not saved and the order status has somehow been set to NULL. The result is that the order order is no longer visible in the default view, although it can be recovered by searching for it by order number and changing the status back to something sensible. One would guess that this is something to do with user authority, but it is odd that the order status is changed, and I am told (I have not seen it myself) that the admin user can carry on working without needing to log in again.
