How to charge a fee for one item plus X for more items

My store sells multiple products and I need a way to charge customers a fixed rate for one item for shipping plus x amount for each additional item ordered.
Example: Customer orders one product only they would be charged $5.45 for shipping. But if the customer orderes two products then they should be charged $5.45 for the first item plus $0.50 for each additional item in thier shopping cart at checkout.

Does anyone know how to do this?

0.50 per product flat rate + handling fee of 4.95

Does flat.php have a handling fee input box?  I cannot remember.
