Hello All, hopefully someone has seen this problem and provide some assistance.
Both Payment mention show up without any problem. See the image below:

When Credit Card "Radio" button is clicked, instead of getting TextBox field, I see blanks. Please see the image below.

Has anyone encounter similar problem. I have double checked all the files and doesn't seem like anything was missed.

very strange as fields are missing, the easiest way to fix is to download other module and try.
