Everytime I try to use UPS shipping, I get these errors..

and so on and so on... I get about twenty of them refering back to link 138, 139, 140, 138, 139, 140, etc..
Actually, I can't seem to get ANY of the shipping modules working properly... anyone have any ideas what I'm (most probably) doing wrong?
